Losing the booklet does not erase the pet’s veterinary history, but that history must be recovered from primary evidence. Scan the microchip, contact the clinic that vaccinated the pet, request copies of its register or medical file and ask the sampling veterinarian and laboratory about the titre report. A new passport must not contain entries reconstructed from memory: every transferred procedure needs a traceable source.
Start by identifying exactly what is missing
Establish whether the missing item is the veterinary passport, laboratory report, Animal Health Certificate, Ukrainian movement document or the whole file. Check email, messages and cloud storage: photographs may help identify the issuing body and reference details, although they do not always replace an original.
Have the microchip physically scanned and record the code. A number recalled from memory or copied from an old advertisement is not sufficient evidence.
Recovering veterinary-passport records
Contact the clinic that microchipped or vaccinated the pet. Ask it to search by owner, chip code, procedure date or product batch and explain its official process for a replacement document or confirmation.
Entries in a new passport should be transferred by an authorised veterinarian from a register, medical record, stock record or other primary evidence. Reattaching stickers or recreating stamps yourself makes the document unreliable.
A lost titre report or certificate
For a laboratory report, contact both the veterinarian who collected the sample and the laboratory. Provide the microchip number, sampling date and owner details. Only the laboratory or authorised institution can confirm the result or issue an acceptable duplicate under its procedures.
An Animal Health Certificate is issued for a specific movement. If it is lost or damaged, immediately contact the authority that issued or endorsed it; an ordinary photocopy does not guarantee acceptance at entry.
Checks after records are restored
- the chip code matches across the new passport, laboratory confirmation and certificate;
- every transferred entry has a source, signature and issuing-body details;
- the vaccination history contains no unexplained gaps or conflicting dates;
- owner and accompanying-person details are consistent;
- the competent authority has confirmed which originals the route requires.
Protecting the file in future
Make colour scans of every page and store them in protected cloud storage. Keep separate contact details for the clinic, laboratory and certificate-issuing authority.
A digital copy assists recovery, but it does not mean the pet can travel without originals where the rules require their presentation.
